About the Role
We're looking for a Service Coordinator to support the delivery of a flexible, community-based home support service for older people. In this role, you'll work collaboratively to ensure services meet individual needs and promote independence in the community. This is a 12 month role covering maternity leave
Responsibilities
- Support the multidisciplinary team to deliver high-quality community support
- Complete clinical assessments for non-complex clients
- Build strong relationships with clients, whanau, and health professionals
About You
- Registered health professional (RN, OT, Physio) with current NZ practicing certificate
- Minimum three years post-registration experience in aged care or community health
- Demonstrated ability to develop person-centered support plans
