In every aspect of our work, we are committed to helping whānau be the best they can be. **He Kōrero Mō Te Tūranga | The Roles** We have one Support Worker role available in the Hutt Valley. **Permanent Part-Time - Access & Choice Service (20 hours/week, Mon-Fri)** Community Support Worker’s with Access and Choice aim to connect directly with whaiora (service users) who are referred via their GP practice to provide brief intervention. This involves working alongside whaiora at their GP practice to develop and complete an identified goal for up to three months to improve their health and wellbeing. This role is walking alongside whanau to give them the tools, confidence, knowledge, and guidance of their community. For example, navigating social services, developing personal plans, gaining employment, budgeting, and housing. You will also be an integral part of the general practice team and working in collaboration with other community partners, as well as building new relationships through networking. This is a 20 hour a week position working Monday –Friday with flexibility to be working weekends **He Kōrero Mōu | About You** We want to hear from people who are passionate about making a real difference and who bring empathy, respect, and cultural awareness to their mahi.
