* Full time, but would consider 0.8 FTE if that works better for you * Mon to Fri 8.00am - 4.30pm with some flexibility to work from home * A company car to use for work purposes, plus a mobile phone and laptop About the organisation This nationwide organisation is a leading provider of home-based healthcare and support. They support people to remain active, safe and independent in their own homes and community. About the role Based on the restorative model of care, your goal as a community nurse is to work holistically alongside the people you support and their families; encouraging the highest level of independence possible. This includes: * Being based in the Timaru office looking after the geographical region of Timaru and surrounds including Geraldine and Waimate * Undertaking social and clinical assessments * Developing, overseeing and regularly reviewing support plans (InterRai) * Mentoring, verifying and supervising support workers * Working closely with RN, Care Coordinator and Support Worker colleagues * Liaising with funders and other providers * An evolving caseload, covering various contracts. * Less hands on clinical nursing tasks and more holistic care coordination * Full Time 8am - 4.30pm (will consider 32 hours per week). About you * A wonderful communicator - able to develop trust and relationships easily - with clients, family members, key referrer organisations and colleagues * You'll enjoy autonomy and working independently but naturally, want to contribute to the team and work closely with others * Adaptable and flexible. Whilst the days and hours are set, you'll find that no two days are the same * Well-organised with excellent time management skills, yet able to change priorities and adapt the schedule with ease * Technically savvy - you'll be working online and offline remotely with a laptop and phone while on the go * Full Driver's License Due to the autonomous nature of this role it won't suit a graduate or nurse with limited New Zealand experience. Ideally you will have worked autonomously in the home and community care sector before or have District Nursing, LTC or medical nursing experience.
