Community Nurse

About the role Based on the restorative model of care, your goal as a community nurse is to work holistically alongside the people you support and their families; encouraging the highest level of independence possible. This includes: * Being based in the Timaru office looking after the geographical region of Timaru and surrounds including Geraldine and Waimate * Undertaking social and clinical assessments * Developing, overseeing and regularly reviewing support plans (InterRai) * Mentoring, verifying and supervising support workers * Working closely with RN, Care Coordinator and Support Worker colleagues * Liaising with funders and other providers * An evolving caseload, covering various contracts. * Less hands on clinical nursing tasks and more holistic care coordination * Full Time 8am - 4.30pm (will consider 32 hours per week). About you * A wonderful communicator - able to develop trust and relationships easily - with clients, family members, key referrer organisations and colleagues * You'll enjoy autonomy and working independently but naturally, want to contribute to the team and work closely with others * Adaptable and flexible. Whilst the days and hours are set, you'll find that no two days are the same * Well-organised with excellent time management skills, yet able to change priorities and adapt the schedule with ease * Technically savvy - you'll be working online and offline remotely with a laptop and phone while on the go * Full Driver's License Due to the autonomous nature of this role it won't suit a graduate or nurse with limited New Zealand experience. Ideally you will have worked autonomously in the home and community care sector before or have District Nursing, LTC or medical nursing experience. Mō te tūranga | About the role The responsibility of Kaiwhakarite is to ensure the front desk welcomes manuhiri/whānau positively and delivers all administrative tasks to the highest quality standards. They will enter and process all incoming referrals for triage, and complete data entry to a high standard. Key responsibilities: • Greet and welcome manuhiri and whānau on arrival. • Have excellent communication and interpersonal skills. • Demonstrate empathy and respect for others. • Be able to deliver the highest standard of customer service. • Have excellent attention to detail and a high level of accuracy. • Be competent in the use of Microsoft Office suite. • Be innovative and open to change. • Performs well under pressure. • Have a fun ‘can do’ personality and works well in a team. • Experience in healthcare preferred. • Answer, screen, and forward incoming phone calls. • Ensure the reception area is tidy and presentable, with all necessary stationery and material. • Provide basic and accurate information in person, via phone and email. • Receive or collect, sort, and distribute mail and deliveries. • Order office supplies and maintain inventory of stock that includes stationery, hygiene, cleaning products, and kitchen supplies for all Arowhenua Whānau Service locations. • Update calendars and schedule meetings as required. • Perform clerical duties that include filing, photocopying, and or transcribing, or other tasks asked by Kaihautū, Pou Whakahaere, Pūmanawa Kaihautū and Kaiwhakahaere. • Organising workplace orientation for all new kaimahi starting at Arowhenua Whānau Services. • Organise equipment, and stationery for new kaimahi such as keys, phone and phone number, laptop, desk, chair, and business cards. • Coordinate and manage orientation documentation alongside the Pūmanawa kaihautū, and Health and Safety (Nga Puna Hauora). • Provide administrative support to the Pūmanawa Kaihautū and for recruitment processes. • Monitor and update Arowhenua Whānau Services kaimahi of all changes to policies and procedures and ensure all relevant forms and applications are completed in a timely manner. • Handle, manage, and direct inbound and outbound calls related to referrals. • Proactively engage with manuhiri, whānau, and key stakeholders and respond promptly to inquiries accordingly. • Process referrals and service requests effectively and efficiently with agreed timeframes and with a high level of data accuracy and speed. • Enter data into the Client/Patient management system accurately. Essential professional qualifications: • Relevant knowledge and experience working in Reception and Administration. • Relevant qualifications and or experience in working within the health and social services sectors. Other essential requirements: • Hold a current NZ full driver’s licence. • First Aid Certificate or willingness to work towards one.
