Description

Job title: Dementia Navigator Division/Department: Pacific Dementia - Māngalo Reports to: Dementia Project Manager Key Relationships: Dementia Project Manager, Dementia Clinical Lead, Dementia Clinicians Essential Duties and Responsibilities: Program Development and Planning: a. Collaborate with the team leader and other stakeholders to develop a comprehensive post-diagnostic support and navigation program for individuals with dementia and their families. b. Identify the specific needs and challenges faced by Pacific communities in accessing dementia support services. c. Design culturally appropriate strategies and interventions to address these needs and challenges. Community Engagement and Outreach: a. Build and maintain partnerships with Pacific communities, community leaders, advocacy groups, and other relevant stakeholders to raise awareness about dementia and available support services. b. Organise and participate in community events, workshops, and educational sessions to disseminate information about dementia and promote early detection and intervention. c. Collaborate with community organizations to develop culturally appropriate educational materials and resources on dementia care and support. Service Delivery and Coordination: a. Oversee the delivery of post-diagnostic support services, including information provision, emotional support, care planning, and access to appropriate resources and services. b. Coordinate with healthcare professionals, social services, community organizations, and other stakeholders to ensure a seamless and integrated support system for individuals with dementia. c. Monitor and evaluate the quality and outcomes of the support services provided, making necessary adjustments to improve effectiveness. Data Collection and Reporting: a. Establish a system for data collection and reporting to track the progress and impact of the post-diagnostic support and navigation program. b. Collect and analyse data on service utilization, client outcomes, and satisfaction levels to inform program improvements and demonstrate effectiveness. c. Prepare regular reports and presentations for senior management and funding agencies to showcase the achievements and impact of the program. Continuous Professional Development: a. Stay updated on the latest research, best practices, and policies related to dementia care and support. b. Attend relevant workshops and training programs to enhance knowledge and skills in dementia care and program management. c. Engage in networking opportunities with professionals in the field to exchange ideas and stay informed about emerging trends. Policy and Advocacy: a. Stay informed about local, regional, and national policies and initiatives related to dementia care and support. b. Advocate for the needs and rights of individuals with dementia and their families, actively participating in policy discussions and influencing decision-making processes. c. Collaborate with other organizations and stakeholders to advocate for improved access to dementia support services and resources. Documentation and Compliance: a. Ensure accurate and up-to-date documentation of client interactions, assessments, care plans, and service referrals. b. Adhere to ethical and legal standards in maintaining client confidentiality and privacy. c. Comply with all relevant regulations, policies, and procedures governing the provision of dementia support services. * Perform related duties as assigned by Dementia Lead Performance Navigator * Maintain compliance with all company policies and procedures Education and/or Work Experience Requirements: * Excellent verbal and written communication skills, with the ability to communicate effectively with both internal and external stakeholders. * Strong computer proficiency, including Microsoft Office (Word, Excel, Outlook). * Ability to work well under pressure and meet deadlines while maintaining a positive attitude and delivering high-quality customer service. * Demonstrated ability to work independently and complete assignments accurately within established guidelines, routines, and accepted standards. * A minimum of three years of relevant work experience is required. * A bachelor’s degree in a health-related field is desirable and will be considered an advantage. * Bilingual proficiency of English and Tongan and deep understanding of Tonga's culture and customs are required for this position. Physical Requirements: * Ability to safely and successfully perform the essential job functions consistent with the New Zealand Standards, including meeting qualitative and/or quantitative productivity standards. * Ability to maintain regular, punctual attendance consistent with the New Zealand Standards and Legislations * Must be able to talk, listen and speak clearly on telephone
